54/y/o man presents complaining of a severe headache. It started last night but has rapidly become more intense. He describes the headache as generalized type. This has occurred thrice in last 2 months. Sometimes it is associated with drowsiness and confusion. He finds bright lights uncomfortable. There is no significant previous medical history On Examination Temperature is 99.5°f. He has stiffness rotation of neck. His eardrums appear normal. BP 110/74 mmHg.
