Shared a case on Curofy
A 23 yr old medical student came with a pyogenic granuloma on his right thumb just distal to the first metacarpal joint. The granuloma was excised and 4 sutures were taken. Prophylactic antibiotics (Amox-Clav 625) was given. However he developed infection with pus collection and after 5 days the stitches were removed and the whole area was cleaned.
Currently it appears to be an ulcer (shown in the diagram).
Note : No restriction in movement of thumb. No compromise in blood supply to thumb.
Mode of treatment?
His query : On consulting a plastic surgeon, he was asked to leave the wound open and apply only mupirocin ointment and to clean the would with soap and water. He was doubtful about leaving the ulcer open!
